Common Vision Issues
Here are some common vision issues that our screenings test for:
Astigmatism
Astigmatism is a common eye condition that causes blurry or distorted vision at all distances, resulting from an irregular shape of the cornea or lens. It happens when the front surface of the eye is shaped more like an egg or a football than a round basketball
Myopia (Nearsightedness)
Nearsightedness, also called myopia, is a common eye condition where you can see things close to you clearly, but things far away look blurry
Hyperopia (Farsightedness)
Farsightedness, also called Hyperopia, is a common vision condition where close objects appear blurry while distant objects may remain clearer

Our screening tests for refractive errors such as Myopia, Hyperopia, Astigmatism, and Anisometria.
Screening also assesses eye alignment and physical errors such as strabismus, anisocoria, and media opacities.

PROCEDURAL GUIDELINES FOR THE VOUCHER APPLICATION AND AWARD
The voucher application itself is housed on StoryLionsClub.com. Individuals seeking assistance are encouraged to print and complete the application in full, including the date and signature. The applicant will return the voucher application via USPS to Story Lion’s Club, P.O. Box 3, Story, WY 8284. Once received, the designated Lion will request a brief interview with the applicant for review of need. If the applicant agrees, two Lions will be present for the interaction.
The completed application, along with the Lion recommendation, will be moved forward to the membership body for a vote on support or denial.
NOTE: Before a vote, there is no assurance of approval. The interview and completed application must both be in place before it is moved to membership consideration.
There are two considerations for which applicants are advised regarding potential financial assistance: (A) If funds are available in the regional clubs that have invested in the program. (B) Financial support that could be available to the applicant from family members and/or other cooperative relationships within the community.
Voucher applications approved by committee vote will bear an affixed official LIONS CLUB Emblem sticker indicating official authorization for payment of $250.00. ANY expenses beyond this sum will be the responsibility of the applicant.
A Lion representative will notify applicants if their a[application has been approved or not. If approved, the applicant will receive a list of local eye care specialists to schedule an appointment. The Lions Club will then send the completed, authorized voucher directly to the specialist the applicant selects. Once the appointment is complete, the local professional vision specialist will then send the invoice to Story Lions Club, P.O. Box 3, Story, WY 82842, for payment.
Story Lions Club Treasurer will complete the VOUCHER payment.
A designated Lion will keep records of the number of Vision Screening Referrals and the number of Vouchers that are awarded to better forecast budgeting needs as the program continues to develop.
Recommendation for awarded applicants: It is appropriate for the individual directly receiving financial assistance to make an acknowledgment through a thank-you note to: Story Lions Club, PO Box 3, Story, WY 82842
My child recieved a referral reccomendation during the screening, what do I do now??
An eye exam and glasses can be expensive; we might be able to help.
The Story Lions Club has partnered with other Sheridan area Lions Clubs and our local Salvation Army in order to fund eye care for kids in our community.
If your child qualifies for free or reduced lunch program options at school, they may qualify!
Each voucher is worth $250.00 towards eye care at local clinics.
